The title of the book took its inspiration from the literary work of William Shakespeare called Julius Caesar, specifically Act 1, Scene 2 of the said literary piece.

The teenagers met in a support group for Hazel Grace is currently suffering from thyroid cancer that has impacted her lungs while Augustus is a former basketball player with an amputated leg.
